Essential Information about our Office
Insurance Accepted
Blue Cross Blue Shield, United Health/UBH, Aetna, Humana, Humana Military, Cigna, Magellan, Beacon Health, Medicare, Worker’s Compensation (for evaluation but not treatment), Tricare Retiree, Tricare Reserved Select, Tricare Standard, some other minor companies and out-of-network benefits.
* Please note we are not a Medicaid Provider
Payment
Checks, cash, and VISA accepted and due at the time of service.
Cancellation Policy
If you cancel an appointment with fewer than 24 hours notice, you (not your insurance provider) will be charged for the cancelled appointment. . If our office determines that the reason for the cancellation is an emergency (I.e. a death in the family, hospitalization, etc.), their will be no charge. We cannot charge your insurance companies for your missed appointment.
Missed Appointments
We understand that our patients lead very busy lives, but if you simply miss or forget about your appointment, the same charge will be applied.
Hours of Operation
Monday – Thursday: 8:00am-12:00pm, 1:00pm-5:00pm. Friday: 8:00am-4:00pm
